What To Expect During Your First Visit
1. When you call to make your appointment, there will be a non-refundable $50.00 deposit required to hold your appointment. It will be credited to your account upon completion of your initial visit.
2. Please bring your ID and insurance card: make sure you call and check your Chiropractic Benefits prior to your visit
3. You will complete an initial assessment and health history form. You can print off the forms from here and have them filled out.
4. Dr. Lockhart will review your assessment form and health history with you.
5. An examination will be completed, that includes but not limited to: Orthopedic test, range of motion and a posture assessments, blood pressure, and a spinal examination.
6. Dr. Lockhart will review the examination findings with you and depending on the findings you will either:
a. Receive an adjustment
b. Referred for images
c. Referred to appropriate office
Payments
Payments are expected at the time of service. We offer a variety of payment options and a time of service discount for those paying cash.
We are in network with certain insurance companies. You can call your insurance provider to check your chiropractic benefits, or give us a call to find out if we take your insurance.
** With insurances that we are a non-participating provider, payment is expected at the time of service. We will submit your claim to your insurance, and your insurance will reimburse you what they deem necessary**
